Dear Valued Patient,

We hope this notification finds you in good health. You are receiving this notice because our files indicate that you are/will become eligible for Medicare. If you have already been seen in our practice using your “original” Medicare card, within the last three years, this letter does not impact your relationship with Nye Partners. If you have selected a Medicare Replacement plan, commonly known as Medicare Part C or Medicare Advantage Plan this notification does not affect your relationship with our practice. We will continue to provide you with the highest level of care.

Due to Congressional rules, regulations, and low reimbursement imposed on medical providers, we find it necessary to close our Medicare panel to all NEW “Original” Medicare members, to remain financially solvent. We are no longer accepting NEW “Original” Medicare (red/white/blue card) patients. We will continue to accept Medicare replacement members, better known as Medicare Part C
